Source-backed working pathway
To establish you'll probably need:
These steps translate the cited manual section into a practical review sequence. Confirm the current local process before acting.
Describe the accessibility barrier with the support coordinator.
Identify the home or vehicle barrier, the health or independence problem it creates, and the change being considered so the need can be addressed in the person-centered service plan.
Obtain an occupational-therapy or physical-therapy recommendation.
The manual requires every adaptation to be recommended by an occupational therapist or physical therapist. Installation plans must be coordinated with that therapist.
Collect at least two bids or cost estimates.
The manual says bids or estimates are obtained from two or more providers chosen by the individual. Authorization uses the lowest and best reasonable price within the service limit.
Confirm the contractor or vehicle modifier meets the provider rules.
The provider must hold a current Department of Mental Health contract, a valid business license, and qualifications for the applicable codes, construction standards, and vehicle-safety standards.
Confirm authorization before work begins.
Ask the support coordinator or authorizing staff to confirm the approved scope, provider, amount, documentation, and completion requirements. The manual does not publish one statewide consumer intake form for this service.
Important boundaries
Limits that may affect fit or authorization
- General home improvements without a direct medical or remedial benefit are excluded.
- The service generally cannot add square footage except when an addition is necessary to complete an adaptation.
- A provider-owned or provider-leased home or vehicle cannot be adapted through this service.
- The service does not purchase or lease a vehicle and does not cover ordinary vehicle upkeep.
- The manual lists a $20,000 maximum every two years per individual, including consultation; current authorization and limits must be confirmed before relying on that amount.
- Comprehensive and Community Support waivers remain under a temporary renewal extension through December 27, 2026, or until CMS grants final approval. Confirm the currently approved rules with the support coordinator; proposed renewal changes are not automatically active.
