Source-backed working pathway

To establish you'll probably need:

These steps translate the cited manual section into a practical review sequence. Confirm the current local process before acting.

  1. Identify the item, supply, or repair and its direct benefit.

With the support coordinator, describe the device or supply, the activity-of-daily-living, communication, environmental-control, life-support, medical, or repair need it addresses, and how that need is reflected in the person-centered service plan.

  2. Check Medicaid and age-specific coverage first.

Determine whether Missouri HealthNet durable-medical-equipment coverage, Healthy Children and Youth services, pre-certification, the Missouri HealthNet exceptions process, or another state-plan route covers the item. For ages 4 through 20, the manual directs incontinence-supply requests through the children's coverage and DME pre-certification process.

  3. Obtain the prescription, clinical information, or prior authorization the coverage route requires.

For state-plan durable medical equipment or supplies, confirm which enrolled prescriber must initiate the request and which enrolled equipment provider completes it. For a waiver-funded request, ask authorizing staff what assessment, order, denial, or other documentation is required.

  4. Collect at least two itemized bids or estimates.

The manual requires estimates from two or more providers chosen by the person. The authorized amount is based on the lowest and best reasonable price within the applicable service limit.

  5. Confirm the current $10,000 annual limit and authorization before ordering.

Current Division guidance lists one unit per month per provider and a $10,000 maximum per annual person-centered service-plan year, per individual. Confirm the person's remaining authorized amount, selected provider, approved item and amount, proof-of-delivery requirements, setup or fitting, refill process, and billing start date before ordering.

Important boundaries

Limits that may affect fit or authorization

  • Medicaid state-plan coverage, including Healthy Children and Youth coverage when applicable, must be accessed first. Waiver funds are for additional qualifying items or repairs that are not otherwise covered.
  • The manual describes different coverage pathways for incontinence supplies based on age and waiver status. The correct Missouri HealthNet, pre-certification, exception, or waiver route must be confirmed before ordering.
  • Two or more bids or cost estimates are required, and authorization uses the lowest and best reasonable price within the applicable limit.
  • Current Division guidance lists one unit per month per provider and a $10,000 maximum per annual person-centered service-plan year, per individual. The waiver manual still contains older $7,500 language, but the Division's 2026 clarification says the manual will be updated and identifies $10,000 as the current maximum. Current authorization and remaining funds must still be confirmed before ordering.
  • Providers must obtain and keep proof of delivery before billing. Items that require setup, fitting, or instruction cannot be billed before those steps occur and are documented.
  • Recurring supplies cannot be shipped automatically. The provider must receive a current refill request and may contact the person or caregiver no sooner than 14 calendar days before delivery or shipping.
  • Comprehensive and Community Support waivers remain under a temporary renewal extension through December 27, 2026, or until CMS grants final approval. Confirm the currently approved rules with the support coordinator; proposed renewal changes are not automatically active.

Questions to confirm locally

  • Is the item covered through Missouri HealthNet durable medical equipment, Healthy Children and Youth, pre-certification, the exceptions process, or another state-plan route before waiver funding is considered?
  • Which prescriber, assessment, order, denial, or supporting documentation is required?
  • Which enrolled providers can supply the item and provide two comparable itemized bids?
  • How much of the current $10,000 annual maximum is already authorized or spent, and how much remains available for this request?
  • What proof of delivery, fitting, setup, instruction, warranty, repair, and record-retention documents will be required?
  • For recurring supplies, how will the provider obtain and document each timely refill request without automatic shipping?
